What is your cancellation policy? 

  • Please cancel appointments 48 hours before the appointed time.   When you schedule an appointment, you are reserving that provider's time and, in the process, guaranteeing that you will be present for the appointment.  If an appointment is canceled late or a patient fails to keep an appointment, the full cost of the visit will be charged to your credit card held on file.  If an appointment is canceled without 48 hours notice or an appointment canceled the same day, you will be charged $75-$290, depending on your provider.  If there is an emergency, please notify our staff as soon as possible.  


What about medication refills? 

  • Due to changing state and federal regulations, medication refills and/or changes can only be done during a scheduled appointment time.  Should you need a medication change outside of your appointment, requests will only be done during business hours, and there may be a charge assessed and added to your account.  Please note this office DOES NOT honor refill requests issued by a pharmacy.  ALL MEDICATION REQUESTS will require 3-5 business days for processing.  You should request a refill or reschedule your next appointment when you have one week of medication left. ​​ 


What is the patient's responsibility with medications? 

  • It is important to continue therapy and any medications your provider has prescribed for you EXACTLY as they have been prescribed.  Please do not skip doses or alter your dosage without provider instruction.  This can be dangerous with any medication. If you require a 90-day supply of medication for insurance to cover it, it is your responsibility to inform your provider of this during your appointment.  


Does the clinic take insurance? 

  • Our Psychiatric Nurse Practitioners, accept a variety of insurances.  Insurance can be confusing and involves co-pays, co-insurance, usual and customary charges, deductibles, and allowables.  Because every plan is different and benefits vary according to policies, you are responsible for confirming that we are participating in your plan and the services are covered.  It is your responsibility to make the clinic aware of any changes in your contact information, such as phone numbers and addresses, as well as any insurance changes. 
  • Dr. Byrd, in his clinic, does not accept insurance of any form.  


What is the no-show policy?

  • If you fail to show up for an appointment or do not give the office 48 hours notice before canceling, then you can be charged (see cancellation policy for more info). 
  • In the event that you have three or more NO SHOWS for scheduled appointments, the clinic reserves the right to discharge you from treatment.  In this case, after the account is settled, we will forward your medical records to a provider of your choice. You will be given a 30 day supply of current medication to give you time to find another provider.


How can I request my records? 

  • Records requests may take 2-4 weeks to complete.  All requests require a release form signed and on file in this office before releasing records.  All requests for records will be assessed a $25 fee. 


What if I cannot afford the cost of my visit?  

  • Payment is due when services are rendered.  Any co-pay, deductible amounts, self-pay rates, etc., are the patient's responsibility at the time of service.  If you cannot pay your scheduled appointment fees, you must reschedule your appointment. 
  • We also are willing to work with you through one of our third-party vendors, Advance Care.  If you are unable to pay your account balance in full, please contact us to set up a payment plan.  While we make every effort to resolve issues regarding payment from your health insurance company, please remember that you are ultimately responsible for the payment for your healthcare services.  If arrangements are not made to satisfy an outstanding balance, or if a payment is not made within 3 months on the account, we may place your account with a collection agency, take legal action to obtain payment, or deny future services.  


What is a UDS? 

  • A Urine Drug Screen is required by law every 90 days for patients taking controlled substances.  The in-office fee for this is $30 per screening.  Patients with abnormal tests will (at our discretion) require a discussion of the current treatment plan and/or modifications to it.
